Terms and Conditions

The undersigned(Member), desires to become a member of Rapid Fitness, and agrees to the following terms and conditions:
PAYMENT OF MEMBERSHIP DUES. The Member agrees to pay monthly membership dues, plus applicable taxes, each month thereafter through automatic withdrawals from an account maintained in a financial institution. Additionally, the member shall pay this agreement, an initiation fee of Ten Dollars($10), unless otherwise stated(redeeming a special).

The Member agrees to pay membership dues of ______ plus applicable taxes on the monthly first, every month until the membership is terminated(refer to the cancellation policy). If the Member has signed up for a 12-month agreement, I agree to pay at least 12 monthly payments. At the end of the 12-Month Agreement, the contract will continue on a month-to-month basis. If the Member terminates a 12-Month agreement before 12 Monthly Payments have been collected, the Member agrees to pay $15 for every month their Membership has been paid for. Enrollment Cost is a one time fee of $30+tax for the first Member and additional Members would be a one time $10 per person.
CANCELLATION POLICY. Automatic drafts will not be canceled without fifteen(15) days written notice, which must be filled out at the Rapid Fitness front desk.
RELEASE AND WAIVER OF LIABILITY. The Member/Members recognizes that there are hazards and risks connected with physical activity. The Member assumes the risks, which include, but are not limited to, fainting, heart attack, dehydration, muscle strains, stress fractures, back pain and more. The Member agrees to defend, indemnify and hold harmless Rapid Fitness against any loss, damage or expense incurred by reason of any claim or liability based upon personal injury(including death). The Member further agrees to release Rapid Fitness and its owners, officers, agents, employees and or affiliates from any and all liability arising out of injury to Member.
SURVEILLANCE/UNAUTHORIZED ACCESS. I acknowledge that surveillance cameras are in use for the protection of the facility, its equipment and members. If I allow unauthorized access I agree to pay a fee of $20 per person per offense.
ACCEPTANCE OF TERMS. As a Member, I understand that I am entitled to use the Facility within the scope of the membership I have selected, and I am obligated to pay my dues and fees regardless of whether or not I use the Facility.
NO REFUNDS. All sales are final at Rapid Fitness. There are no refunds, even if a member chooses not to use the facility or services of the club.
GUEST POLICY. If you have a friend or family member that wants to use the facility, just contact Rapid Fitness staff for a day or week pass.
GYM BASICS. Chalk is amazing, just dont make a mess. Heavy weights are amazing, just put them away when youre done. Gym selfies are amazing in great lighting, just dont photograph other members. Sweating will happen, just wipe your equipment when youre finished with it. Be courteous of your neighborit could be their first time in a gym.
BROKEN EQUIPMENT. If you find a piece of equipment that is broken, please bring it to the attention of the staff immediately so we can keep everything in safe, working condition.
I certify that I have read the foregoing Membership Agreement, and by agreeing to the terms and conditions, I acknowledge that I understand and agree to be bound by all of the terms and conditions hereof. I further acknowledge that a fully executed copy of this Membership Agreement has been provided to me.
